This form is provided as an example of how to record
your CE activities throughout the year and simplify reporting each
year.

SUGGESTIONS FOR PERSONAL CE RECORD KEEPING
Maintaining records of continuing education (CE) activities
is the responsibility of individual members. It is only necessary
to report the number of credits earned under the Association-Defined
Component to the OPFA. However, a member may want to record all
CE activities in the case of disciplinary action or peer review
of continuing education participation.
How to remember what I took?
You may develop your own system of record-keeping, which is fine.
Whatever way works for you works for us, as long as there is some
form of paper trail.
The minimum suggested is a summary of the CE activities
you participated in for a given year. Keeping a copy of the
agenda from workshops, conferences, courses, etc., is a useful way
to remember what you have attended and for determining the number
of applicable CE credits.
For reading articles and books, it is suggested you
maintain a list using standard referencing form (see examples below)
so any reviewer will be able to search out the particular item.
Standard Reference form:
Author(s) (surname, first initials, separated by semi-colons), Year
of Publication, Full Title, Publishing Authority, Community, Province/State,
Country (if out of Canada), Report Number (for numbered reports)
or ISBN Number or Volume/Issue and Pages.
